Hello.
I recently started again and Its been pretty tough lol. I know alot of things have changed and made the game a little bit easier then it used to be. This is my 3rd time around playing this game. My first time playing was from 2001-2004 which ended up getting banned i wont go into details right now.
My second account was from 2006-2008, this account was hacked and never returned, after this happend I had called up Square Enix and they sent me a form to fill out, and also to get notarized before mailing out, which I did. I never recieved an email or any replies, so by then I already turned over to a new game which kept my interest away from FFXI.
I am now currently playing on a new account with the same name of my previous accounts characters had, I'm just a bit discuraged that I would have to start all over again. I am on my 2nd week of my free month and have been having a blast just enjoying the game however I know i'd have more fun with one of my old accounts =D.
I guess my post here was to see if anyone here ever had a problem like this and if they ever got their account back. I would like to talk with someone at Square Enix if either one of my 1st two accounts are possible to be returned. I might have lost the CD keys throughout the years and I know this maybe a problem. I can answer any questions they ask about my previous accounts so I just hope maybe I can get some answers.

A friend of mine had his account hacked. Lucky for him, I was online when it happened and was able to track activity on his account. His character was leader of a party in Gusgen mines(everyone else in the area was under 20). Nothing unusual, as my friend is helpful. But he wasn't answering my /tell's. Then my phone rings, and my friend informs me that he can't access his account, that he got kicked. I called a GM and was able to suspend the account temporarily.
Anyways, the short of it is, my friend got his account back very quickly because he had me, a real life friend, to help him. He got lucky that the hacker didn't have time to part with his items or gil.
My point is, if you have a real life friend who plays with you, it's as good as never losing your account.

same happened to a friend of mine, his account was hacked, and was signed to gm immediately. everybody who knew him saw it was a stranger on his account (different behaving, combat style and so on). all i can say is : get friends to prevent getting hacked and being lost again, comfort

Did you happen to make any copies of the information you sent to SE? Your paperwork probably got lost in the mail or ended up in the bottom of a pile on someone's desk that no longer works for SE. Unfortunately, paperwork gets lost all the time in corporations (trust me, I work for one and know all the problems that can happen usually due to lazy employees) and you should always back up your documents.
On a different note, I've had a few friends that have lost their accounts due to being banned or hacked. SE never allows a banned account to be reopened, at least from what my friends said to me. SE is also EXTREMELY protective of people's account information due to privacy laws. You can verify everything about your account with SE but if you don't have the proper documentation, you won't get any answers.
