In the Compact Keyboard Type it might be easier if the macro ctrl keys were mapped to the number keys. It would be more efficient and comfortable just pressing the number keys without the ctrl. The ctrl keys could then be used to change between macro pages in a book. Just a thought!

How would you handle the ALT-row?

How is pushing the ctrl/alt on a compact any more difficult than on a full size keyboard?

here's a better idea, keep ctrl/alt macros the same and open up 1-0 to be used as stand alone macros like ffxiv

I think I would keep the alt row keys. They are more comfortable to press than the ctrl row keys I find. You could always remove them, but that would limit the number of macros I would imagine, right?

Oh there is no difference in difficulty, but in a compact keyboard layout the number keys are not assigned to anything. In a full size keyboard pressing the number keys opens the chat window... Unless they've changed that. I haven't used a full size keyboard for FFXI in a while now!
