Steps:
I have completed both the rank 2 hunting journal for the Flames and the quest Shadows Uncast, however the Personnel Officer tells me that I need to talk to the Flame Commander for approval before being allowed to be promoted. Flame Commander Swift has no additional dialog or quests for me.
I have attempted logging out and back in to fix it, but this did not change anything.

I'm having the same problem with the Twin Adders. I'm Chief Serpent Sergeant, and the NPC says the same thing, but the Commander has no dialog.

A day later the commander randomly had the quest Twisted Beliefs for me, I am working on completing it now, will report back what happens afterwards.

Completed dungeon and purchased promotion. Not sure what was wrong last night.
