Wanted: vengeance

Hiie,

so me and some of my fellow LS member we try to get all of us the gears, so we been doing Operation: Crosseye and so .. the ogre is regenerating HP but somehow we able to kill them, but unable to obtain any gold chest afterwards .. so those LS members whos first timer go and do the second and third faction leve for gears, where they found there problem/bug/glitch where after they complete all 3 quests, the faction leve "Wanted: Vengeance" did not unlock to them

i have done this several times with different kind of groups and peoples, but this is the first time i encounter this issue, i search the net and seems no result

so i wonder if someone could look into this problem?

thanks Qeepel for the reply, sorry about the template will make sure i posted it accordingly next time. anyway this issues been occurring on few of my LS member, i will ensure they fill the template and posted it in here ASAP
