    Appcrash / DirectX crash

    Details:
I sent a GTX 460 in to have the fans repaired a while ago. While that was gone, I had been using a 9800GTX+. When I had the GTX 460, and while using the 9800GTX+, my game played fine without any sort of unsual crash. After I got the GTX 460 back, I swapped that out and started having problems. Initially, the computer was locking up and required a reboot, but that stopped after updating to a newer set of beta graphics card drivers. With the new drivers, however, the problem shifted from the computer locking up to the game crashing, with either the generic "DirectX device has stopped working" error or an appcrash error. The specific appcrash that I've been getting is:

    Problem signature:
    Problem Event Name: APPCRASH
    Application Name: ffxivgame.exe
    Application Version: 1.0.0.0
    Application Timestamp: 5023e45e
    Fault Module Name: ffxivgame.exe
    Fault Module Version: 1.0.0.0
    Fault Module Timestamp: 5023e45e
    Exception Code: c0000005
    Exception Offset: 00647985
    OS Version: 6.1.7601.2.1.0.256.1
    Locale ID: 1033
    Additional Information 1: 0a9e
    Additional Information 2: 0a9e372d3b4ad19135b953a78882e789
    Additional Information 3: 0a9e
    Additional Information 4: 0a9e372d3b4ad19135b953a78882e789
I've tried just about everything I could think of: updating all drivers, trying older drivers, reinstalling the game, reinstalling windows 7, downclocking the card, et c. That same specific appcrash happens when using the GTX 460, regardless of drivers and other variables. I haven't encountered any problems with other games or programs, and have been able to stress test the card for both performance and power consumption without a problem.

Lowering the settings of the game seems to make the crashes less frequent, and downclocking it doesn't seem to have any effect. I haven't encountered the error at all using the 9800GTX+, either.

Date & Time: Since 23.8.12 and ongoing
Frequency: Always when using the GTX 460
PC Manufacturer & Model Number: Custom
    OS: Windows 7 Ultimate
    Graphics Card: GTX 460
