What is going on with my hotbars??

I just recently transferred two of my characters to another server, and ever since then one of them has been having an issue where every time I log off, cleanly I might add, and then log back on all but one of them get completely reset and I have to reorganize them, this is getting very frustrating and I would like it fixed, please.

I have heard of this, but if you are on the PC, you can remove the settings files for your character that links to the one that is having trouble and reset everything up. Then it should save and stick.

As for the PS4, I am not sure where those files are saved.
