You can trade the equipment/materia/catalyst to a crafter and have them attach it for you.
This one isn't the most rigorous test but I realized I have two different high level weapons with +10 enmity on them (second worst T4) and one is an HQ so I checked the difference in success rate between and was getting a +2.00% increase on the HQ vs. the NQ. Trying to add a second best T4 materia (an Accuracy+18), I got 15.50% on an NQ Demilune Bhuj and 17.50% on a +1 Cobalt Winglet. More data soon.
I've been assembling a lot more stats and the preliminary finding is that you get higher success rates by using lower tier materia first. Example:
Existing materia T4-2/4, forbidden materia T3-3/4, success rate: 18.0%
Existing materia T3-4/4, forbidden materia T4-3/4, success rate: 20.0%
The second example is a more powerful combination of materia but has a higher success rate, hypothesis being that the strength of the existing materia has a bigger negative impact on success rate than the strength of the materia being added. I'll keep analyzing data and post again if anything new solidifies.

I have a complete table of forbidden success rates for combining Tier III and Tier IV materia.
* bolded numbers have all been verified in-game; non-bold numbers are inferred but assumed correct
Select the column of the first materia inserted and the row of the materia to be added. For example, if you already have a 1/4 roll Tier IV, adding a 4/4 Tier IV will have a 16.00% chance. Add +2% for HQ1, and +3% for HQ2. I don't know about HQ3, since I don't have any +3 equipment that can be melded.
If you don't know which roll your materia is, check the materia list on ffxivpro. Each materia there will list 16 bonus numbers. There are 4 rolls per tier, so Tier IV materia will be the 13th-16th numbers. For example, Tier IV Bloodthirst can be 55, 60, 65, or 70, so I refer to a +55 HP Bloodthirst as a "Tier IV, 1/4" on the table.
