First thing, turn off Ambient Occlusion if it is on, and then turn your General Draw Quality to 8 (if it is above 8). Those two settings cause 90% of the FPS issues in high-end systems.

With AO off, and draw quality at 8, I was able to max out -every- other setting with NO problems on a i5 quad/GTX460. There is no significant difference between General Draw Quality 8 and 10, and even less difference with 16x AA

As I know, this game doesn't perform very well with AMD and ATI comparing to Intel and GT.
Try to update your driver and lower your game configuration.

Yes, forcing vsync is the solution, at the expense of 1-2 fps.

    0. close FFXIV and go to your nvidia control panel

    1. click on manage 3D settings

    2. add ffxivgame.exe to the list

    3. scroll down untill you see the vsync option and set it to "force on"

    4. click apply, restart your FFXIV client

The tearing should be gone now. It works fine for me.

This is what I found on FFXIVcore site, it may help, search on google, you may find some other advise.
