As for the issue, the random crashes did stop after that one day, however the headset-unplugging is still causing a crash 100% of the time....

Its the Aoe-Placement change SE made. Any targeted aoe (turrets, Flaming Arrow, Shadowflare etc) will, when macroed, be placed under the <t>, or in your case, on top of you.
