Shadows on the enemy could have absorbed the first (and strongest) hit, then a secondary hit might have been partially absorbed by a stoneskin effect, or the secondary hit might just not have been a critical hit.
It's also possible that you simply missed the first hit. Hit rate caps at 95% you know.

A 192 damage blade hi is cause by missing the first 2 hits and having only a double attack hit connect, and that hit not criting. Or by being a very terrible ninja, but this sort of thing is more common then you think, acc caps at 95%, so you always have a chance to miss both of those 2 initial hits no matter what gear you use.

Double digits depending on the mob. It's a pretty random weapon skill sometimes.
Yeah, This WS is both the best and the worst ever. one time on a Bluffalo I hit for 73... and then the same monster for 4500 xD
I don't think it is much more random than other similar WSes. Steel Cyclone would act in the same way if it wasn't for the fact that a second Double Attack hit from a greataxe is stronger than a single hit from a katana.
Oh and except that SC isn't a crit WS so it doesn't normally get that huge numbers anyway.

Blade: HI is too Random >.<
ODD doesn't affect weaponskills.
