So after some time off from FFXI I decided to come back. So much has really changed since I left

So my basic question would be - whats the thing to do to level? Am I a newb if I just put my flag up? What access to areas do I need?

Well, really, Abyssea access is the big thing for leveling.

For good or ill, what happens a lot now, is leveling a job to 30ish, and then leeching experience in Abyssea alliances until the mid-70s (sometimes all the way to 90), and then doing a massive skillup spree to become useful.
